Create a discrepancy log that preserves every name exactly as issued. Distinguish a spelling or transliteration difference from a documented legal name change before asking how to present it.

What the official guidance establishes

UK passport guidance requires the passport name to match travel bookings. Canada’s IMM 5669 instructions require names as shown in the passport, travel document or identity document; these are examples of why the receiving authority’s wording matters. Read GOV.UK: passport names and changes for the original scope and current instructions.

Inventory names without correcting the evidence

Make a row for each passport, civil record, qualification and application record. Copy the displayed spelling, script and order faithfully. Add a separate note explaining the difference you see. Do not retouch a scan or rewrite a historical certificate to make the rows match.

Connect genuine changes to their records

For a previous or changed name, identify the actual document linking the versions. For a transliteration issue, keep both the original script and the existing translation. Mark any proposed explanation as a draft until the intended recipient confirms how it should be presented.

Ask one precise question

State the application, the two name versions and the records concerned. Ask whether an explanation, corrected record or specific supporting document is required. Use a secure channel and send only what is necessary for that question. Preserve the response for the person preparing the final form.

Your working sheet

This is an optional IMMIGEUROPE planning worksheet, not an official application form or a new document requirement.

Worked planning example

A fictional graduate’s diploma uses a shortened given name while the passport uses the full name. The log records both and asks the recipient how to establish the link. It does not describe the shortened form as a legal name change without evidence.

Use the confirmed explanation consistently while retaining the original records. Keep the next action in the moving planner and use the guide library for the next part of your move.
